Fall 2020 Physicals for Students

SPORTS PHYSICALS 

A physical examination is required each year prior to participation in any athletic activity sponsored by the Nebraska School Activities Association.  The physical examination is valid for one year from the date of the exam and may be performed any time after May 1 prior to the next school year.  Due to Covid-19, there are some exceptions for physicals for the 2020-2021 school year. 

  • All students entering grades 7 and 9 that would like to participate in athletics need to have sports physical for 2020-2021. 
  • Students entering grades 8, 10, 11, or 12:  If they had a sports physical on file at the school for 2019-2020, they do not need a new one.  If they did not have one on file for the 2019-2020 school year, they will need a physical for the 2020-2021 school year.

20-21 Sports Physical Form (Students entering grades 7, 9 and students that did not have a sports physical on file last year need this form.)

20-21 NSAA Parent Consent Form  (ALL STUDENTS, GRADES 7-12, THAT PLAN TO PARTICIPATE IN SPORTS, NEED THIS FORM SIGNED.)

 

SEVENTH GRADE PHYSICAL

A physical exam is required upon entry to 7th grade according to Nebraska LB59 law.  This physical may be performed up to six months prior to the beginning of 7th Grade.  

The clinic is requesting a parent/guardian to please accompany the student to the exam.  If the student plans to participate in sports, the sports physical will be completed at the same time the 7th grade physical is performed.

All 7th grade students are required by DHHS to have three doses of the Hepatitis B vaccine, two doses of MMR vaccine and two doses of the Varicella vaccine or a history of having chicken pox. Immunizations are available at the clinic or for a reduced cost at area immunization clinics.  

Once the physical exam and immunizations are completed, the original physical form(s) and record of immunizations should be returned to the school.  This can be done any time prior to the first day of school.  The cost of this physical is $80.00.  Immunizations are not included in this charge.  

KINDERGARTEN PHYSICAL 

A physical exam is required upon entry to kindergarten according to Nebraska LB59 law.  This physical may be performed up to six months prior to the beginning of kindergarten.

The following immunizations are required by DHHS upon entrance to kindergarten:  two doses of MMR, 3 doses of Hepatitis, at least 3 doses of DPT and IPV, and two doses of the Varicella vaccine (if your child has not had the chicken pox).  Please bring your child’s immunization records at the time of the physical so we can make sure their immunizations are current.  Immunizations are available at the clinic or at area immunization clinics for a reduced cost.

Kindergarten/7th Grade Physical Form

 

 

Physicals are available at Henderson Health Care between 9 a.m. and 5 p.m. Monday through Friday.  Payment is required  at the time of service.
